This is a very short anonymous patient questionnaire that we send to all our patients to monitor their experience of the clinic's services. Thank you very much for your feedback.

If you have any concerns or queries relating to your treatment you can always call the clinic and ask to speak with your chiropractor. If you prefer to email, please click on his/her name below.


If you are not satisfied with any aspect of our service we would like to hear from you. We have procedures in place to ensure your complaint is treated fairly, efficiently and professionally.
